1. A completed and signed enrollment application form

2. A 3040 form and 680 form These documents inform us that your child has received all IMMUNIZATIONS required to date in accordance with his/her age and include a current STATEMENT OF GOOD HEALTH signed by a licensed health professional within the last 24 months (PHYSICAL EXAM).

3. A completed food form which enrolls your child in our food program.

4. A completed infant feeding form, if applicable.

5. A completed Swim Central water safety questionnaire. This form is required by Broward County Childcare Licensing.

6. A fee agreement form that confirms your understanding of all financial responsibilities.

7. A completed family questionnaire. This document allows us to gather information you wish to share about you and your child that will assist us in developing the most appropriate, culture-sensitive environment possible.

8. Schedule a parent orientation meeting with the Site Director for a complete introduction to our program.

9. Signed and complete the Permission to Release Photograph
